Adapting ACT Center materials
The Center for Accelerating Care Transformation (ACT Center) encourages the non-commercial use or adaptation of the materials we develop. With the exception of the Chronic Care Model image, all other materials we produce are publicly available, free of cost, and copyright protected.
The Chronic Care Model
For current research referencing the conception of the Chronic Care Model in the field of chronic illness care, please use this citation: “Wagner EH. Chronic disease management: what will it take to improve care for chronic illness? Eff Clin Pract. 1998;1:2-4.”
The copyright for the Chronic Care Model image is owned by the American College of Physicians (ACP) which publishes the Annals of Internal Medicine, the journal in which the model first appeared. All permitted use of the Chronic Care Model image must be secured through the Copyright Clearance Center. ACP will not charge for use in thesis, dissertation and in most non-commercial cases, but the $0.00 permission must still be secured through the site. This process usually takes 1-2 days. Upon receiving permission, our preferred citation in your work is as follows: “Developed by the Center for Accelerating Care Transformation, formerly known as the MacColl Center for Health Care Innovation, © ACP-ASIM Journals and Books, reprinted with permission from ACP-ASIM Journals and Book.”
