Advancing high-value care by partnering with national, state, and local organizations

Our team is committed to building and sustaining models and systems of care that provide patients with high-quality, safe, efficient, evidence-based care. We advance high-value care by partnering with national, state, and local organizations to explore, develop, and implement sustainable value-based payment models, and to implement interventions to help reduce the use of services for which the potential for harm is greater than the benefit.

The 4 projects below provide in-depth models, tools, and resources for advancing high-value care. The resources were primarily developed and tested in community health centers, but most are applicable to a variety of care settings.

• The Delta Center for a Thriving Safety Net is the first national center to focus exclusively on transforming payment and care for ambulatory primary care and behavioral health services, providing technical assistance to state health associations. 
   
• The Model for Advancing High Performance in Primary Care and Behavioral Health (MAHP 2.0) provides community health centers and community behavioral health organizations with an evidence-based roadmap to improve care, demonstrate value, and succeed under value-based payment arrangements.

• The Framework for Creating a Regional Healthcare System provides a practical model for improving health care and reducing costs through collaborations across geographic areas.

• The Taking Action on Overuse Framework is a model to engage providers, staff, and patients in reducing care that is more likely to cause harm than to provide a benefit.
